Bike hire USA

Am looking at a big road trip in the states at some point, maybe 2009.

Can anybody recommend some bike hire companies out there that do a good deal including insurance etc?
Most hire places I have seen have daily rates, very limited mileage and little or no insurance cover.

An organised trip could be a possibility but I'd rather do my own thing. Different pickup/drop off locations would be ideal.
